Withering Matron, Hag-Lich Matriarch

Old tales call her the Withering Matron. She was once a coven leader who sought immortality and bound her soul to a phylactery made from the bone of an ancient crone. Over centuries she became a nexus of both hag hexcraft and lich necromancy. Villagers whisper she bargains with grief and births curses to be sold as favors.

Spellcasting. The hag-lich is a 9th-level spellcaster. Its spellcasting ability is Intelligence (spell save , to hit with spell attacks). It has 9 1st-level spell slots and the slots listed in the Spells section are its prepared spells.
Necrotic Sustenance. The hag-lich has a hidden set of charms and ritual jars. Whenever it deals necrotic damage to a creature, it regains hit points equal to half the necrotic damage dealt.
Immutable Undead. The hag-lich cannot be turned. Effects that control undead have disadvantage against it.
Hex Cache. Hex Cache. The hag-lich stores up to three active curses and can apply one when it hits with Melee Spellstrike or uses Curse Lash. Minor curses impose small mechanical penalties and major curses impose larger ongoing penalties. A remove curse ends minor curses; greater restoration ends major curses.
Phylactery. Phylactery. When the hag-lich is reduced to 0 hit points, its body collapses into animated bones. If the phylactery is intact the hag-lich reforms in days unless the phylactery is destroyed. Destroying the phylactery requires overcoming a trapped ossuary and a Investigation or Arcana check to find its true bond.
Melee Spellstrike. Melee Spellstrike. The hag-lich makes a melee spell attack with its bone-hand. On a hit, it deals 18 () necrotic piercing damage plus one curse effect from its Hex Cache. The hag-lich chooses the curse when it makes the attack. Target must succeed on a Constitution saving throw or be affected by the curse for 1 minute. A cursed creature can repeat the save at the end of each of its turns, ending the curse on a success.
Energy Drain (Recharge 5-6). Energy Drain. The hag-lich targets one creature it can see within 60 feet. The target must succeed on a Constitution saving throw or take 32 () necrotic damage and the hag-lich regains hit points equal to half the necrotic damage dealt. On a successful save, the target takes half damage and no hit points are restored.
Withering Gaze. Withering Gaze. The hag-lich fixes its hollow eyes on a creature it can see within 30 feet. The target must succeed on a Wisdom saving throw or be frightened and have its maximum hit points reduced by 10 () until the end of the hag-lich's next turn. A creature that succeeds is immune to this hag-lich's Withering Gaze for 24 hours.
Mirror Hex. Mirror Hex. When a creature misses the hag-lich with a melee attack, she can choose to reflect a minor curse back onto that attacker. The attacker must succeed on a Wisdom saving throw or suffer the minor curse for 1 minute.
Curse Lash. Curse Lash. The hag-lich targets one creature within 30 feet. The target must succeed on a Wisdom save or gain one minor curse from the Hex Cache. The curse lasts 1 minute.
Necrotic Spike (Costs 2 Actions). Necrotic Spike. The hag-lich causes spectral thorns to erupt around one creature within 60 feet. The target takes 14 () necrotic damage and must succeed on a Constitution save or have disadvantage on concentration checks until the end of its next turn.
Command Undead (Costs 3 Actions). Command Undead. The hag-lich targets one undead creature within 60 feet that it can see. The target must succeed on a Wisdom save or be charmed and follow a simple command until the end of the hag-lich's next turn.
