Vorath, the Gullet King

Miner lore speaks of the 'Gullet King,' a beast that split the mouths of careless miners and left entire crews paralyzed by a green fog. Old journals advise leaving iron and quicklime along the path to discourage its visits. Superstitious tunnelers sometimes leave offerings — bits of ore or tallow candles — in hopes the Vorath will spare them.

Many-Eyed Vigilance. The Vorath has advantage on Wisdom (Perception) checks that rely on sight because of its many eyes and on checks that rely on smell or vibrations in underground environments.
Segmented Gullet. The Vorath's segmented jaws allow it to bite and hold multiple angles. When the Vorath hits with a Voracious Bite, it can choose to immediately attempt to Grapple the target as a free action, using its Athletics bonus . The Vorath can grapple up to two Large or smaller creatures at once using its multiple tongues and jaws but taking a -2 penalty to each grapple check after the first.
Toxin-Hardened. The Vorath's skin and internal chemistry make it naturally resistant to poisons. It has resistance to poison damage and advantage on saving throws against poison.
Glandular Reserve. The Vorath secretes powerful toxins. Any nonmagical weapon that deals slashing or piercing damage to the Vorath while inside its lair has a 10 percent chance per successful hit of becoming slicked with toxin, requiring the wielder to succeed on a Constitution save or take 3 () poison damage from incidental contact.
Voracious Bite. Melee Weapon Attack: to hit, reach 10 ft., one target. Hit: 18 () piercing damage plus 10 () poison damage. If the target is a creature, it must succeed on a Constitution saving throw or become poisoned for 1 minute. While poisoned in this way the creature is also slowed: its speed is halved and it has disadvantage on Dexterity saving throws. The poisoned creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
Rend and Clamp. Melee Weapon Attack: to hit, reach 5 ft., one Large or smaller creature grappled by the Vorath can be Rend. Hit: 22 () slashing damage. If the target is Medium or smaller, it is also restrained as the Vorath's segmented jaws clamp and the tongues lash around the limbs. A creature restrained by Rend can use an action and succeed on a Strength (Athletics) check to free itself, or another creature can use an action to help free it with the same check.
Rasping Tongues. The Vorath lashes with two long, rasping tongues in a single sweeping motion. Each creature in a 15-foot cone must make a Dexterity saving throw, taking 14 () slashing damage on a failed save, or half as much damage on a successful one. A creature hit by the tongues must also succeed on a Constitution saving throw or be coated with a viscous glandular secretion. A coated creature takes 7 () poison damage at the start of its turn for 3 rounds unless it uses an action to scrape the secretion off ( Dexterity check) or succeeds on the Constitution save.
Glandular Exhalation. The Vorath exhales a short, fetid cloud of toxic mist in a 10-foot radius centered on a point it can see within 30 feet. Each creature in that area must make a Constitution saving throw, taking 21 () poison damage on a failed save, or half as much damage on a success. Objects and unattended gear in the area become slick with toxin but are not otherwise harmed. After using this action the Vorath's toxin glands are spent and it cannot use this feature again until it finishes a short or long rest.
Snapback. When a creature within its reach attempts to move away while grappled by the Vorath, the Vorath makes one bite attack against that creature.
Detect. Detect. The Vorath makes a Wisdom (Perception) check.
Tail Sweep (Costs 2 Actions). Tail Sweep. The Vorath makes a tail attack: Melee Weapon Attack: to hit, reach 10 ft., one creature. Hit: 13 () bludgeoning damage and the target must succeed on a Strength saving throw or be knocked prone.
