Faceless Stalker - AI-generated fantasy Monster

Faceless Stalker

Whispered stories tell of a quiet man who lost his family to fire and was seen no more, only for a faceless shadow to begin leaving bodies with clean chest wounds. Priests claim the Faceless Stalker is the product of an old bargain with a hunger in the void. Grave-keepers and watchmen speak the name only in hisses and crosses at midnight.

Shadow Stealth. While in dim light or darkness the Faceless Stalker has advantage on Dexterity (Stealth) checks and on attack rolls. It can attempt to Hide even when only lightly obscured by shadow.
Faceless. The Stalker has no face. It cannot be surprised in dim light or darkness. Creatures that attempt to read its expression gain no information and have disadvantage on Insight checks against it.
Furious Phase. On reaching 50 hit points or fewer the Faceless Stalker enters Furious Phase. In this phase its attack rolls and damage rolls increase by +2 and Heart Rip becomes available. Its maw opens, adding terrifying presence to its attacks.
Light Vulnerability. If the Faceless Stalker is in bright light it suffers disadvantage on attack rolls and Stealth checks and loses its resistance to nonmagical weapon damage.
Multiattack. The Faceless Stalker makes three Claw attacks, or two Claw attacks and uses Heart Impale if the conditions are met.
Claw. Melee Weapon Attack: to hit, reach 5 ft., one target. Hit: slashing damage. If the Faceless Stalker hits the same creature with two or more Claw attacks on a single turn, that creature must succeed on a Constitution saving throw or become Impaled (restrained by shadow knives). An Impaled creature takes an additional piercing damage immediately and at the start of each of its turns until it succeeds on a Strength check to free itself or the Faceless Stalker removes the impalement as a bonus action. The Faceless Stalker can Impale only one creature at a time.
Heart Rip (Furious Phase only). Melee Weapon Attack: to hit, reach 5 ft., one target. Hit: piercing damage. If the target is Impaled and fails a Constitution saving throw, the Faceless Stalker rips out the target's heart: the target takes an additional necrotic damage and is reduced to 0 hit points if that damage reduces it to 0. A creature reduced to 0 by Heart Rip is dead and cannot be stabilized by normal means until the Faceless Stalker is removed and the corpse is tended with a successful Medicine check or cured by a spell such as raise dead.
Shadow Lash. Ranged Spell Attack: The Faceless Stalker lashes a handlike shadow at a creature within 30 feet. The target must succeed on a Dexterity saving throw or take necrotic damage and have disadvantage on Perception checks relying on sight until the end of its next turn. In dim light or darkness the target also has disadvantage on the saving throw.
Shadow Step. When a creature ends its turn within 5 feet of the Faceless Stalker in dim light or darkness, the Stalker may use Shadow Step. The Stalker disappears into shadow and reappears in an unoccupied space within 30 feet that is in dim light or darkness.
Shadow Skitter. The Faceless Stalker moves up to half its speed without provoking opportunity attacks or makes one Claw attack.
