Eldra's resolve is tied to the Heart. If the party convinces her that the conduits are worsening the blight, she hesitates and loses her next reaction.
Severing the conduits removes chamber hazard options, making the encounter safer even though Eldra remains dangerous.

Blight ConduitsThree conduits cross the chamber. A creature adjacent to a conduit can use an action to make a DC 15 Strength (Athletics) or Dexterity check using thieves' tools, herbalism kit, or a suitable blade. On a success, that conduit is severed. Each severed conduit suppresses one chamber lair effect chosen by the DM. Eldra cannot restore a severed conduit.
Root-Lane CoverRaised roots provide half cover to a creature that is not prone and is adjacent to them. A creature can climb over a raised root at the cost of 5 extra feet of movement.
Rootbound. Eldra has advantage on saving throws against being poisoned, and nonmagical plants cannot restrain her or impede her movement.
Anchored Warden. Eldra has advantage on ability checks and saving throws made to resist being moved or knocked prone while she is touching the ground or a root.
Heart-Sense. While at least one blight conduit remains intact, Eldra knows the location of any creature touching the ground within 60 feet of the Verdant Heart, unless that creature is behind total cover.

Actions

Multiattack. Eldra makes two Briar-Rake attacks.
Briar-Rake. Melee Weapon Attack: to hit, reach 10 ft., one target. Hit: 13 () slashing damage plus 4 () poison damage. If the target is Large or smaller, it must succeed on a Strength saving throw or be pulled up to 10 feet toward Eldra.
Rootline Snare (Recharge 5-6). Eldra lashes a 20-foot-long, 5-foot-wide line with thorned roots. Each creature in the line must make a Dexterity saving throw, taking 22 () piercing damage on a failed save, or half as much on a success. A creature that fails is also restrained by roots. A restrained creature can use its action to make a Strength check, freeing itself on a success. The roots wither after Eldra's next turn.
Black Sap Surge (Recharge 5-6). Eldra calls sap up in a 10-foot-radius pool centered on a point she can see within 60 feet. Each creature in the area must make a Dexterity saving throw. On a failure, it takes 18 () acid damage and falls prone; on a success, it takes half damage and remains standing. The area becomes difficult terrain until the end of Eldra's next turn.

Reactions

Grasping Root. When a creature Eldra can see moves within 10 feet of her, Eldra lashes out with a root. The creature must succeed on a Strength saving throw or have its speed reduced to 0 until the end of the current turn.

Legendary Actions

Rootstep. Eldra moves up to half her speed without provoking opportunity attacks, provided she moves along or across a root lane.
Tangle. One creature Eldra can see within 10 feet must succeed on a Strength saving throw or be pulled 5 feet and have its speed reduced by 10 feet until the end of its next turn.
Rake (costs 2 actions). Eldra makes one Briar-Rake attack.

Notes for the game master

Eldra Voss is a custom creature, not a canonical D&D monster.
The chamber hazards and blight conduit rules are custom encounter mechanics.
Eldra has three legendary actions per round and can use them at the end of another creature's turn. She regains spent legendary actions at the start of her turn.

Tactics

  1. Opening

Eldra speaks from among the raised roots, demanding that the intruders leave the Heart undisturbed. If they refuse or approach the Heart, she attacks from a lane that limits how many characters can reach her at once.

  2. Mid-fight

She uses Rootline Snare to separate the party, then pulls a trapped character into a hazardous lane or toward the sap pool. She uses Briar-Rake against nearby threats and reserves her reaction to punish movement through the roots.

  3. End of the fight

When reduced to 45 hit points or fewer, Eldra prioritizes Black Sap Surge and attacks characters working on the Heart or conduits. If the conduits have been severed, she loses access to the chamber's lair effects and fights without relying on the sap.

  4. Retreat

Eldra does not retreat while the Verdant Heart is threatened. If reduced to 20 hit points or fewer and the Heart is no longer endangered, she offers information or a bargain rather than surrendering her life.

How it fights

Keep Eldra mobile along root lanes instead of letting her stand still and trade attacks.
Describe the conduits clearly so players can make informed choices about spending actions to sever them.
Use the sap pool as a looming danger, not an unavoidable damage source.
Allow characters to exploit cover, ranged attacks, clever movement, and social appeals.

Who it attacks first

A character approaching the Verdant Heart
A character attempting to sever a blight conduit
A character isolated in a root lane
A nearby character concentrating on a spell

Works well with

Root-lane cover and difficult terrain reward Eldra for holding a narrow approach while leaving the party room to maneuver.
The conduit objective gives characters a meaningful alternative to focusing only on damage.
Eldra's pulls and snares can set up the sap pool and the chamber's lair effects.

How to beat it

Severing a blight conduit weakens the chamber hazards and prevents Eldra from using the corresponding lair effect.
Forced movement can pull Eldra out of cover or away from a threatened character, though she can use her reaction to resist some movement.
Ranged attacks and spells let characters threaten her without entering the most constricted root lanes.
Fire is not an automatic weakness, but burning roots can clear a 5-foot square of root cover at the DM's discretion.

Ways to avoid a fight

A character can try to reach the person beneath the transformation with a DC 15 Charisma (Persuasion) check. On a success, Eldra pauses until the end of her next turn unless someone attacks her or touches the Heart.
Characters can examine a conduit with a DC 14 Intelligence (Nature) or Wisdom (Medicine) check to learn that severing it will reduce the chamber's hazards, though it may worsen the Heart's condition.
A successful DC 15 Wisdom (Insight) check reveals Eldra's fear that the Heart will die if the conduits are cut, rather than simple cruelty.

Ecology

Eldra Voss wears a weathered green coat fused to her shoulders by living roots. Fine rootlets web her face, leaving one human eye visible beside a glossy, seed-dark eye. Her fingers have become hooked thorns, and each breath stirs loose leaves that were not there a moment before.

Behavior

Eldra is controlled, grieving, and frighteningly practical. She sees the Heart as a patient under her care and the party as surgeons who have arrived with knives. She fights to keep intruders away from the Heart, not for sport, and briefly hesitates if someone addresses her by name or offers a credible way to save it.

How it lives

Eldra no longer needs ordinary food or sleep. She draws strength from the blighted root network and remains close to the Verdant Heart. Without the Heart's influence, her transformation may slowly recede, though the process is uncertain and painful.

Lair

The heart chamber is a circular hollow beneath the oldest tree. Raised roots create narrow lanes around a pool of dark sap, while the Verdant Heart hangs overhead. On initiative count 20, losing initiative ties, choose one effect while at least one blight conduit remains: a root lane becomes difficult terrain until the next round; a 10-foot patch beside the sap pool releases fumes, requiring a DC 14 Constitution saving throw or dealing 7 (2d6) poison damage; or roots grasp one creature on the floor, requiring a DC 14 Strength saving throw or restraining it until initiative count 20 next round. Each severed conduit removes one of these effects from the DM's options. These are custom encounter rules, not a standard monster ability.

Habitat
The hollow beneath an ancient tree, especially a circular heart chamber with raised root lanes, a dark sap pool, and a suspended Verdant Heart.

Signs that it is near

Pale leaves scattered where no branches reach.
Thin roots twitching beneath the floor when no wind is present.
Dark sap that clings to boots and smells faintly of crushed mint.
A distant voice asking intruders to leave the Heart alone.

Lore

Eldra Voss was the oldest tree's appointed warden, entrusted with tending the Verdant Heart and guarding the paths beneath its roots. When a creeping blight reached the Heart, she bound herself to the root network to contain it. The binding worked, but the blight took root in her as well. Eldra now believes that any attempt to touch the Heart will finish what the blight began. Whether she is right is for the characters to discover.

What drives it

Protect the Verdant Heart at any cost, and keep anyone from severing the conduits that she believes are holding the chamber together.

After the fight

When Eldra falls, the Heart's pulse becomes audible beneath the roots. If the conduits remain intact, the chamber still threatens those who linger. If the characters severed them, the sap drains away and the oldest tree can begin to heal. Eldra may leave behind a last plea, a warning, or a clue about who first corrupted the Heart.

“When a creeping blight reached the Heart, she bound herself to the root network to contain it.”
From its lore

Variants

For a less corrupted Eldra, remove her poison damage and Rootline Snare, reduce her hit points to 105, and make her more willing to negotiate.
For a harsher version, increase hit points to 160 and let an intact conduit empower one lair effect each round, but do not increase her damage as well.

Hooks

Plot hooks

Eldra's old grove-warden records identify who ordered her to bind herself to the Heart.
A character who once received help from Eldra may recognize her voice and have an opportunity to reach her before the first attack.
The blight may have been deliberately introduced through one conduit, leaving a trail beyond the oldest tree.

How to use it

Use Eldra as the central opponent in The Heart's Last Stand, with the characters balancing damage against actions spent severing blight conduits.
Make the conduit checks visible and understandable before combat or during Eldra's opening warning.
For a party of four characters around level 8, use Eldra alone with the chamber hazards. Add minor root thralls only if the party is unusually strong or well-rested.
For a lower-level party, reduce Eldra to 105 hit points and remove one legendary action per round. For a stronger party, add a root thrall or increase her hit points to 160.

Treasure

The grove-warden token, which can open a sealed archive beneath the oldest tree.
A clear green seed from the Heart, if the party saves it. It may be planted to restore a small patch of blighted land.
Eldra's water-stained field journal, containing a clue to the source of the blight.
