Root-and-Water Ambusher. The snake has advantage on Dexterity (Stealth) checks made to hide among roots, submerged debris, or broken stone. In the flooded hollow, it can hold still beneath the water without needing to breathe.
Compelled Watch. The snake has advantage on saving throws against being charmed or frightened while its binding token remains intact.

Actions

Fang Bite. Melee Weapon Attack: to hit, reach 10 ft., one target. Hit: 10 () piercing damage, and the target must succeed on a Constitution saving throw or take 7 () poison damage. On a successful save, the target takes half as much poison damage.
Coiling Constriction. Melee Weapon Attack: to hit, reach 10 ft., one Medium or smaller creature. Hit: 12 () bludgeoning damage, and the target is grappled (escape ). Until the grapple ends, the target is restrained, and the snake cannot constrict another target.
Multiattack. The snake makes one Fang Bite attack and one Coiling Constriction attack. It can replace either attack with a shove attempt against a creature it has grappled.
Root-Snap Sweep. The snake lashes a root or submerged stone with its tail. Each creature of the snake's choice within 5 feet must succeed on a Dexterity saving throw or fall prone. The snake can then move up to 10 feet without provoking opportunity attacks. It can use this action only while in its guarded hollow.

Reactions

Root-Tethered Lunge. When a creature the snake can see moves within 10 feet of it while in the hollow, the snake moves up to 10 feet toward that creature. This movement does not provoke opportunity attacks. The snake cannot use this reaction while restrained.
Role

Designed for four characters of level 3 as a challenging solo guardian

Gear

  • Sseth-Ka binding token (Wondrous object, 25 gp to a collector; its true significance may be worth more)A green stone token set into its brow, carved with a coiled-serpent sigil. It is part of the binding and is not ordinary equipment.

Notes for the game master

Custom monster designed for D&D 5e, using standard 5e action and condition terminology.
The creature is not an official yuan-ti stat block and has no spellcasting.
Knee-deep water is difficult terrain for creatures without a swim speed, unless they can move along the bottom or use another feature to ignore it. Apply the same rule to the snake only if the water is unusually deep or obstructed.
Root cover and the two elevated stone plates are encounter terrain, not part of the creature's statistics.

Tactics

  1. Opening

Remain still among the submerged roots until a creature enters the hollow. Attack from cover, favoring a creature who approaches the gate alone.

  2. Mid-fight

Use Constrict to restrain an isolated target, then bite it. Use Root-Snap Sweep to disrupt clustered enemies or break contact.

  3. End of the fight

If badly injured, it tries to pin one intruder in the water while withdrawing toward the gate. It does not pursue a foe who retreats beyond the hollow.

  4. Retreat

The snake fights while the gate is threatened. If reduced to 15 hit points or fewer and no enemy is near the gate, it retreats into a narrow root-choked fissure.

How it fights

Hide among roots or beneath the water before combat begins.
Use the hollow's cover to close distance, then restrain a target with Coiling Constriction.
Avoid remaining in the open between the two elevated plates.
Use Root-Snap Sweep when multiple enemies cluster near the gate.

Who it attacks first

A creature approaching or touching the sunken gate
An isolated ranged attacker on a stone plate
A creature that has already been injured or separated from allies

Works well with

Root tangles make it easier for the snake to hide and interrupt movement.
A yuan-ti cultist can exploit the snake's restraint attacks to target trapped intruders.
The elevated stone plates let ranged combatants challenge the snake without standing in the water.

How to beat it

Ranged attacks from the elevated stone plates can target it before it reaches the party.
Forced movement can pull it out of the root cover and break its preferred approach.
A successful DC 13 Strength (Athletics) check frees a creature from its constriction.
Poison resistance or immunity substantially reduces the danger of its bite.

Ways to avoid a fight

A character can approach slowly and leave food at the edge of the hollow. With a successful DC 13 Wisdom (Animal Handling) check, the snake pauses long enough for the party to cross without provoking an attack, provided no one threatens the gate.
A character who can communicate in Abyssal or Draconic may recognize that the snake is compelled to guard the threshold rather than hunt freely.
The binding token can be examined with a successful DC 13 Intelligence (Arcana or Religion) check to identify it as a yuan-ti warding charm.

Ecology

A thick, dark-olive jungle snake, nearly four meters long, with gold flecks in its scales and pale bands of living root wrapped around its neck and flanks. A carved green stone charm is fused to its brow. When it raises its head, the bands tighten like reins.

Behavior

Patient and territorial, it imitates a fallen root until prey comes within reach. It attacks intruders but ignores creatures that leave offerings at the gate and do not cross its submerged boundary.

How it lives

The snake survives on fish, frogs, and small mammals trapped in the flooded hollow. Its binding charm sustains its watchfulness but does not remove its need to feed.

Lair

The snake's territory is the flooded hollow before the half-buried stone gate. Root tangles conceal a shallow den beneath the gate's lowest fallen lintel.

Habitat
Overgrown jungle hollows, flooded ruins, and buried yuan-ti sanctuaries. At the encounter site, knietiefes Wasser slows movement, roots provide cover, and two raised stone plates offer firing positions.

Signs that it is near

A thin wake travels through otherwise still knee-deep water.
Fresh scales and shed root fibers cling to the stone plates.
Small prey animals avoid the hollow, while frogs gather at its far edge.
A green gleam briefly shows beneath the water near the buried gate.

Lore

Yuan-ti wardens bound the snake to the sunken gate with a carved token and ritual roots. The binding commands it to guard the threshold, not to serve as a messenger or pursue intruders far from the ruins.

What drives it

Protect the sunken gate and obey the compulsion impressed by its binding token.

After the fight

If slain, the snake's bands loosen and the Sseth-Ka mark beneath its scales fades. Its remains can be examined for a small stone token bearing the same sigil as the half-buried gate.

Variants

A weakened guardian has 45 hit points and loses Root-Snap Sweep.
A veteran guardian has 75 hit points and its Fang Bite deals 2d8 + 3 piercing damage, but use this version only for a stronger party.

Hooks

Plot hooks

The token bears a mark matching carvings deeper inside the gate, linking the guardian to the rites of Sseth-Ka.
The snake's binding is weakening. Its changing behavior may indicate that someone has disturbed the wards beyond the gate.
A yuan-ti agent wants the token recovered intact, but removing it may free the snake or trigger a dormant defense.

How to use it

Use as the principal guardian for Wache am versunkenen Tor.
For a party of four characters around level 3, present it alone with the terrain described in the encounter.
For a party of level 4 or higher, add lesser hazards or another weak guardian rather than increasing this creature's damage.
Allow characters to notice its unnatural root bands or the stillness of the water before it attacks.

Treasure

The Sseth-Ka binding token set into the snake's brow.
A watertight pouch in the den containing 18 gp, a tarnished silver ring, and a fragment of a map to the ruins beyond the gate.
