Pack Predator. The creature has advantage on attack rolls against a creature if at least one of the creature's allies is within 5 feet of the target and the ally isn't incapacitated. This functions similarly to Pack Tactics.
Corrosive Ichor. The beast's yellow tentacles exude a corrosive ichor. Any nonmagical weapon or armor that takes acid damage from the tentacles suffers a cumulative -1 penalty to attack rolls or AC respectively for each hit during the encounter, to a maximum penalty of -3. Magical equipment is unaffected.
Mutated Resilience. If the beast starts its turn at half its hit points or less, it regains 5 hit points. This regeneration is suppressed for 1 minute if the beast takes radiant damage.
Gnarled Visage. As a bonus action the beast can attempt to intimidate a single creature within 20 feet that can see it. That creature must succeed on a Wisdom saving throw or be frightened until the end of its next turn. Creatures immune to being frightened are unaffected.

Actions

Rending Bite. Melee Weapon Attack: to hit, reach 5 ft., one target. Hit: 14 () piercing damage. If the target is a creature with Intelligence 6 or higher, it must succeed on a Wisdom saving throw or be disoriented until the end of its next turn. A disoriented creature has disadvantage on attack rolls and ability checks.
Tentacle Lash. Melee Weapon Attack: to hit, reach 10 ft., one target. Hit: 11 () slashing damage plus 7 () acid damage as the yellow tentacle exudes a corrosive ichor. If the target is wearing nonmagical armor, that armor takes a cumulative -1 penalty to AC until the end of the encounter for each time it is hit by a tentacle (to a minimum penalty of -3).
Crushing Sprawl. The chaos beast lashes out at up to three different targets it can see within 15 ft. Each target must succeed on a Dexterity saving throw or take 9 () bludgeoning damage and be knocked prone. On a success, the target takes half damage and is not knocked prone.

Reactions

Protective Frenzy. When a creature the chaos beast can see within 10 ft. makes a melee attack against an ally of the beast, the beast can use its reaction to make a Tentacle Lash attack against that attacker.

Notes for the game master

Designed for DND 5E as a CR 5 minion-style fiend. Uses pack tactics and group coordination to present a dangerous but not overwhelming threat to a party of four to five 5th-level characters.

Tactics

  1. Opening

Rush forward on all six legs, use Crushing Sprawl to scatter the party and follow with Tentacle Lash to apply acid and degrade armor. If a spellcaster or weak-willed target is nearby, attempt to isolate them.

  2. Mid-fight

Expose tentacles to maximize reach, use Crushing Sprawl to knock down multiple foes, then close to finish prone targets with Rending Bite. Take advantage of allies to flank and trigger Pack Tactics bonuses.

  3. End of the fight

If it is dominating a single target, it tries to tear them apart with Rending Bite while other beasts or allies finish off weakened foes. If significantly wounded, it focuses on spreading its corrosive ichor onto the ground to hinder pursuit and buys time for escape.

  4. Retreat

When reduced to about one third of its hit points it will attempt to disengage and flee into planar rifts or dense terrain, using yellow tentacles to smear ichor behind it that slows pursuers. If isolated and cornered, it fights ferociously to the death.

How it fights

Use terrain and ambush points to force piecemeal engagement.
Target spellcasters and weaker-willed characters first to reduce enemy cohesion.
Exploit Pack Tactics equivalents by coordinating attacks with allied beasts.

Who it attacks first

Spellcasters
Healers
Isolated or flanked characters

Works well with

Works with spellcasters that create difficult terrain to maximize Crushing Sprawl impact.
Pairs well with fiendish brutes that can hold the line while the beasts flank and harry casters.

How to beat it

Area control effects to deny movement and clustering.
Spells or abilities that infest or restrain - the beast relies on mobility.
Cold or radiant damage tend to harm it more than fire.

Ways to avoid a fight

Tracking the hardened ichor for alchemical study.
Using ritual cleansing or planar binding to contain a budding rift.
Negotiating with the cultists who summon them to learn of the Snoke of Chaos.

Ecology

A small, hunched fiend roughly the size of a large dog with six jointed, insectile legs and a torso that still hints at humanoid origins. Dozens of slim, yellow tentacles sprout across its shoulders and jawline. Its mouth is a vertical maw filled with layered, needle-sharp teeth. Its skin is mottled crimson and oil-black, veined with faint, sickly yellow light that pulses with every breath.

Behavior

Savage, mocking, and opportunistic. The chaos beast favors harassment, hit-and-run tactics, and targets that inspire fear or cruelty. It is bold in groups and clever enough to exploit cover and obstacles with its many limbs.

How it lives

Born where planar thresholds bleed together, these beasts are often the corrupted remnants of humanoid bullies who trafficked in chaotic rites. Their spawn hunt in small packs, drawn to places where the weave of reality is thin. They feed on fear and flesh, and their ichor alters ecosystems by encouraging fungal and spore growth keyed to chaos.

Lair

A localized planar scar where thin strands of other realms coil into tunnels. The lair is strewn with jagged shards of glasslike residue and bubbling pools of yellow ichor. Ambient space is distorted so that maps and divination are unreliable while inside. The beasts make tunnels between exposed nodes and erupt into the chamber when prey disturbs the rift. The lair grants the beasts advantage on Stealth checks to ambush and allows one Tentacle Lash per round as a lair action against intruders.

Habitat
Planar rupture zones, warped forests, the edges of chaotic demiplanes, ruins corrupted by cultist rituals.

Signs that it is near

Yellow streaks of ichor across walls and ground.
Hollowed, chewed remains with precise, needle-like tooth marks.
A low chorus of wet clicking and indistinct whispers in the dark.

Lore

Stories say these creatures are the result of bullies who bargained with a misnamed trickster known only as the Snoke of Chaos. The bargains turned them inward, stripping their empathy and replacing it with predatory hunger and the yellow madness that chars memories. Cultists prize their remains for rites that bend will.

What drives it

To feed, spread corruption, and obey the faint, cruel commands of planar patrons who cherish discord.

After the fight

The yellow tentacles secrete a stinking, sticky fluid that coats the ground and anything nearby. Within an hour the fluid hardens into brittle, glassy residue that can be mined for small shimmering flakes worth a modest sum to planar alchemists and chaotic cultists. Corpses touched by the creature's ichor sometimes twitch for minutes afterward and can be used to craft unstable reagents or attract more chaos beasts.
