Crossing the threshold wakes the eyes, which fire in a deliberate sequence and mark each target with a different infernal sentence. Stealth
(expert) to notice the eyes' magical nature and avoid drawing their attention; creatures that enter the room trigger the hazard regardless of whether they noticed it.
Trigger
A creature crosses the threshold into the room, or a creature inside the room takes an action that manipulates one of the eyes or the vault's central dais.
Disable
A creature adjacent to an eye can use 2 actions to attempt a Thievery, Occultism, or Religion check to disable it. A critical success disables that eye permanently; a success suppresses it for 1 minute; a failure has no effect; a critical failure causes the eye to fire immediately at the creature. A creature can also disable an eye by destroying it. All eight eyes must be disabled or destroyed to end the hazard.
Initiative
Perception +38
Routine
On its turn, the Witness uses 3 actions. Each action fires the next eye in sequence at one creature in the room, prioritizing creatures that entered most recently and then creatures closest to the center. If multiple creatures are equally eligible, choose randomly. Each eye fires only once before the sequence repeats. The target attempts the save listed for that eye's beam. If no creature is in the room, the hazard holds its actions.
Reset
The hazard resets immediately after its routine, even if no creature remains in the room. It remains active until all eight eyes are disabled or destroyed. A disabled eye reactivates after 1 minute unless critically disabled or destroyed.

Special rules

Eightfold Sequence. The eight eyes fire in a fixed order: acid, cold, electricity, fire, force, mental, sonic, then void. The currently active eye's iris brightens and its sigil turns toward the target before the beam fires, providing a clear warning.

Threshold Witness. The hazard activates when a creature crosses the threshold into the room. It rolls initiative immediately, and the creature that triggered it is a valid target for its first beam. Creatures already in the room when the hazard is activated are also valid targets.

Infernal Beam. Each beam is a magical ranged effect with a range of 120 feet and requires line of effect. It can target only one creature. A creature behind total cover cannot be targeted.

Stop it

Signs of dangerA faint, dry clicking comes from the walls, like eight clocks attempting to agree. · The threshold is etched with tiny silhouettes of people, each with a different mark burned over its eyes. · The floor bears eight shallow grooves aimed toward a central brass drain. · A creature examining the room notices that dust avoids the straight lines between each eye and the center.

Countermeasures

In short

Avoid entering the room, stay behind total cover, or disable the eyes individually.

Its story

What it looks like

Eight enormous eyes are set into the room's walls, one in the center of each face of the octagon.

Why it is here

To deter unauthorized entry and punish intruders with a rotating sequence of infernal judgments.

Who made it

An infernal archivist whose name was struck from every contract it ever wrote.

Lore

Academia Lore or Infernal Lore to Recall Knowledge: The Eightfold Witness is a vault defense designed to punish unauthorized entry while preserving the contents of its chamber.

Where it is found

Vault

Variants

A tribunal version substitutes spirit damage for void damage and causes the beam to reveal one secret the target is trying to conceal on a failed save.
A merciful version deals nonlethal damage and teleports intruders back through the entrance on a critical failure against the force beam.
A corrupted version fires all eight beams in order without pausing, then becomes inert for 1 minute while its eyes weep molten brass.

Plot hooks

The vault contains a prisoner who can speak only when the eyes are firing, because the beams briefly disrupt the infernal binding on their cell.
One eye has been replaced with a mortal eye. Its owner is still alive somewhere and experiences every activation as a flash of pain.
A succession dispute hinges on proving who can cross the room without being branded by the Witness.
The vault's true owner wants the party to retrieve a contract from inside, but refuses to reveal which of the eight eyes recognizes their seal.
