Memory Guardian - AI-generated fantasy Monster

Memory Guardian

Memory Guardians were originally created by a forgotten deity of truth to prevent the manipulation of historical records. Over millennia, some have become corrupted, guarding secrets that should have been shared, or protecting the memories of tyrants.

Spectral Resistance. The Memory Guardian is resistant to all damage types except Radiant.
Incorporeal Movement. The guardian can move through other creatures and objects as if they were difficult terrain. It takes 5 () force damage if it ends its turn inside an object.
Archive Sense. The guardian automatically knows the location of any creature that has touched a memory vessel or scroll within the archive in the last hour.
Multiattack. The Memory Guardian makes two attacks: one with Memory Syphon and one with Phase.
Memory Syphon. The guardian targets one creature it can see within 60 feet. The target must make a Intelligence saving throw. On a failed save, the target takes 21 () psychic damage and the guardian gains 21 temporary hit points. On a success, the damage is halved and no temporary hit points are gained.
Phase. The guardian moves up to its speed. During this movement, it can pass through any number of creatures. Each creature the guardian passes through must succeed on a Charisma saving throw. On a failure, the creature takes 15 necrotic damage and its hit point maximum is reduced by an amount equal to the damage taken. This reduction lasts until the target finishes a long rest.
Scream of Sorrow (Recharge 5-6). The guardian releases a piercing, ethereal wail of pure grief. Each creature within 30 feet of the guardian that can hear it must succeed on a Wisdom saving throw or be Paralyzed for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
Cognitive Blur. When a creature the guardian can see targets it with an attack, the guardian can force the attacker to make a Intelligence saving throw. On a failure, the attack misses as the attacker momentarily forgets where the guardian is located.
