The Sanguine Reliquary Guardian
An ornate basalt sarcophagus stands in the center of the vault, connected to floor channels that form a ritualistic star. The air vibrates with a low, thrumming hum that resonates in the teeth of those nearby.

Detection
“The stone is etched with jagged runes that glow with a faint, pulse-like crimson light. Fresh blood seems to seep from the seams of the lid, flowing perfectly into narrow floor grooves that surround the pedestal.”
Standard Engagement
A creature that does not speak the cult's passphrase or wear their medallion enters a 15-foot radius around the sarcophagus.
(3 actions) The guardian uses its first action to pulse with necrotic energy, triggering Siphon Life. It uses its remaining two actions to lash out with Gore-Streaked Tendrils, targeting up to two different creatures within 15 feet.
The hazard resets after 1 hour as the blood recedes into the stone and the internal mechanisms click back into place.
The sarcophagus releases a wave of crimson mist. All living creatures within 30 feet must attempt a DC 21 basic Fortitude save. On a failure, they take 2d6 necrotic damage and the hazard gains 5 temporary hit points.
Melee attack +14 (reach 15 feet); Damage 2d8+6 bludgeoning plus Grab. The hazard can have up to two creatures grabbed at once.
Whenever a creature takes bleed damage within 30 feet of the Reliquary, the hazard gains a +1 status bonus to its attack rolls and its save DCs for 1 round.
