The Malice-Heart Citadel
A sentient fortress constructed from the petrified remains of a fallen deity. The citadel does not just house traps; it is the trap. It feeds on the biological chemical markers of fear and the psychic resonance of pain. It possesses a cold, calculating intellect that seeks to maximize the 'yield' of its victims by isolating them, presenting false hopes, and manifesting their specific phobias as physical entities.

“The walls are made of a dark, obsidian-like stone that feels warm and slightly yielding to the touch, like pressurized flesh. Veins of violet energy pulse beneath the floorboards, and the architectural style shifts from gothic arches to jagged, alien geometries as the 'mood' of the building changes. Doors look like clenched eyelids, and the air smells of ozone and old blood.”

The Citadel enters its active hunting phase when a creature inside takes more than 50 damage in a single turn or falls under the Frightened condition.
On initiative 20, the Citadel takes three actions: 1. Shifting Maw: It rearranges two rooms or hallways, forcing a DC 22 Dexterity save or a creature is teleported into a hazard room. 2. Manifest Agony: It creates two 'Echos of Pain' (treat as Wraiths) in the nearest combat zone. 3. Psychic Siphon: Any creature currently Frightened or below half HP takes 22 (4d10) necrotic damage, and the Citadel regains that many hit points in one of its Nerve Clusters.
The Citadel is never truly 'spent.' If not destroyed, it resets its layout every dawn. If its Nerve Clusters are destroyed, it goes dormant for 1d100 years.
As a reaction when a creature attempts to move, the walls close in. The creature must make a DC 22 Strength saving throw or be Restrained (escape DC 22) and take 33 (6d10) bludgeoning damage.
The Citadel exhales a purple mist. Each creature in a 60-foot radius must make a DC 22 Wisdom saving throw or lose their lowest-level remaining spell slot or one use of a limited-use class feature.
The Citadel has an Intelligence of 18 and a Wisdom of 20. It can communicate telepathically with anyone inside it, often using the voices of their loved ones to lure them into traps.
The interior of the Citadel is considered difficult terrain for any creature the Citadel chooses to hinder.
