The Defiled Wyrm-Casket
The gargantuan skull of a black dragon, half-sunken in the mire, serves as the focus for a botched resurrection ritual. Instead of reviving the dragon, the cultists bound their own souls into a churning vortex of necrotic bile that overflows from the skull's jaw.

Detection
“A mossy dragon skull the size of a wagon sits tilted in the muck. Violet liquid drips from its fangs, and a sickly green light flickers deep within the hollow eye sockets. Runes carved into the bone are weeping black ichor.”
Standard Engagement
A creature approaches within 15 feet of the skull or touches any of the ritual components lying in the mud.
The hazard takes 3 actions on its turn. Action 1: Churning Miasma. The skull exhales a 15-foot cone of necrotic fog. Creatures in the area must succeed at a DC 16 Fortitude save or become sickened 1. Action 2 and 3: Grasping Ooze. The violet bile flowing from the mouth lashes out like a tongue at one creature within 20 feet. (Melee Strike +10, 1d10+4 void damage plus Grab).
The haunt falls dormant 1 minute after all creatures leave its 20-foot radius. It resets completely at midnight.
On a successful Strike with its tongue, the target is coated in acidic, necrotic bile. They take 1d4 persistent acid damage while grabbed.
When reduced to 0 HP, the skull shatters in an explosion of bone shards. Creatures within 10 feet take 2d6 piercing damage (DC 18 basic Reflex save).
If a creature is killed by the hazard, their soul is temporarily trapped. They cannot be resurrected until the skull is permanently purified with a consecrate spell or similar ritual.
